Job Title: Physical Therapist Home Health Profession: Therapy/Rehabilitation Specialty: Physical Therapy Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 1 year required License: Valid State License required at time of submission Certifications: BLS required; Wound Care preferred Must-Have: - Independent case management skills - Time management and organizational skills - Strong communication skills - Ability to supervise Physical Therapy Assistants - Positive attitude and professionalism Description: This position involves providing home health physical therapy to adult and geriatric patients. The daily caseload includes 5 to 6 units. Therapists will manage patients with diagnoses related to orthopedics, cardiac, neurological, pulmonary conditions, and diabetes, among others. The role requires a patient-centered approach and the ability to set and evaluate patient goals. Clinicians must demonstrate flexibility and a capability to work as part of a multidisciplinary team. Knowledge of adaptive devices and assistive equipment is essential. Therapists will be responsible for patient education and safety. The use of electronic charting is required. Therapists should prepare for potential weekend shifts every other weekend. Traveling professionals will need to float within the service line. All candidates must be willing to provide excellent quality care.
